HPV 26 Positive: Risk Classification and Test Interpretation

Op. Dr. Alparslan Demir · Obstetrics and Gynaecology · Şişli, Istanbul

An HPV 26 positive result does not diagnose cancer. HPV 26 is classified by IARC in Group 2B. Its significance should be assessed using the test panel, Pap findings and previous cervical results, rather than the type number alone.

Is HPV 26 a high-risk type?

IARC Group 2B means possibly carcinogenic to humans, with limited evidence for cervical cancer for this type. It does not state that you have cancer or predict an individual percentage risk. HPV 26 should not be treated as equivalent to HPV 16 or 18 in either evidence strength or management. A laboratory’s high-risk label and an IARC classification may serve different purposes.

Why is HPV 26 listed on one test but absent on another?

HPV assays do not all examine the same types. Common cervical high-risk screening panels and broader genotyping panels can have different coverage. If a report does not mention HPV 26, the type may not have been tested at all; absence from the report is not necessarily a negative result. Compare the assay name, sampling site and full list of types before interpreting changes.

How does my Pap result affect the assessment?

HPV testing detects a viral type, while cytology examines cells. A normal Pap result means no relevant cellular abnormality was identified in that sample; it does not prove that HPV is absent. Abnormal cytology or previous significant cervical disease can be more important for decisions than the genotype alone. There is no responsible single procedure or universal review date to assign from HPV 26 positivity in isolation.

Do I need colposcopy or LEEP?

HPV 26 alone is not a sufficient reason to remove cervical tissue. The need for colposcopy and biopsy depends on cytology, examination, other detected types and previous results. A pathway developed specifically for HPV 16/18 should not automatically be transferred to HPV 26. Symptoms or suspicious findings need clinical assessment even when routine screening would not yet be due.

What about vaccination and the next appointment?

HPV 26 is not one of the nine types directly targeted by the nine-valent vaccine. Vaccination is not a treatment for an existing HPV 26 infection; possible protection against other types is a separate discussion. Bring the full report and earlier HPV, Pap and biopsy records. This allows panel differences, a newly detected result and repeated positivity to be distinguished before agreeing on follow-up.
