HPV 73 Positive: Pap Results, Colposcopy and CIN History

Op. Dr. Alparslan Demir · Obstetrics and Gynaecology · Şişli, Istanbul

HPV 73 positive is a genotyping finding, not a diagnosis of CIN, cancer or a need for surgery. IARC classifies this type in Group 2B. Pap results and any previous cervical biopsy findings are essential context.

What is the risk classification of HPV 73?

IARC places HPV 73 in Group 2B, meaning possibly carcinogenic to humans. This does not mean the evidence or clinical pathway is identical to HPV 16/18. Different risk labels used by older publications or laboratories can be confusing. Interpretation should consider why the test was performed and all accompanying findings, rather than a label alone.

Is HPV 73 the same as CIN?

No. An HPV result identifies a viral type. CIN describes cervical tissue changes assessed through histology. The grade of a cervical lesion cannot be read from an HPV 73 result. Nor does a single positive test justify a cervical procedure. Cytology, colposcopy and biopsy answer different clinical questions and are used when appropriate to the overall assessment.

Would I need colposcopy with a normal Pap result?

A normal Pap result means no relevant cellular abnormality was seen in the sample. Colposcopy decisions are not made from that fact or the HPV 73 number in isolation. Other high-risk types, previous abnormal screening, examination findings and immune conditions may matter. A pathway validated for one screening assay should not automatically be applied to a broad panel with different coverage.

What if I previously had CIN or LEEP?

The previous lesion grade and treatment records are important. Follow-up may differ from that of someone with a first positive HPV result and no relevant history. The clinician considers old pathology, margin information and subsequent tests together. New HPV 73 positivity alone does not prove recurrence of the earlier lesion, but it should not lead to ignoring that history either.

What are the next steps and does vaccination cover it?

The nine-valent vaccine does not directly target HPV 73 and is not treatment for an established infection. Possible preventive benefit against other types is discussed individually. Bring the full HPV panel, Pap report and previous pathology records to the same consultation. After confirming sampling site and assay coverage, appropriate follow-up or investigation can be planned. A universal review interval cannot be assigned from this isolated genotype result.
