How can HPV be positive if the Pap smear is normal?

The tests answer different questions. HPV testing looks for viral material; a Pap smear examines cervical cells. It is therefore possible to detect high-risk HPV while the cells look normal at that moment.

Key point: “Pap normal” does not mean the HPV result is false, and “HPV positive” does not mean cancer is present.

Does every HPV-positive patient need colposcopy?

No. Management depends on genotype, screening method, current cytology, previous results and treatment history. HPV 16 and HPV 18 are managed differently from several other genotypes in current risk-based guidance.

Why HPV 16 and HPV 18 are different

When HPV 16 or HPV 18 is detected, colposcopy is generally recommended even when cytology is negative, because genotype-specific risk is high enough to justify closer assessment.

What if my report says “other high-risk HPV”?

The phrase may represent several genotypes. If the laboratory does not report them individually, do not assume which type is present. The next step may be repeat HPV-based testing, cytology, dual-stain triage or colposcopy depending on the validated test and your history.

What if I had CIN2/3 or LEEP before?

Post-treatment surveillance is different from routine first-time screening. A normal Pap smear does not erase a previous high-grade lesion. Bring the pathology and treatment records.
