HPV 45 Positive: Risk and Follow-up | Şişli, Nişantaşı, Istanbul

Quick answer: HPV 45 is a high-risk genotype with a recognised association with glandular cervical disease. It is biologically related to HPV 18 but is not managed identically in every screening algorithm.

What does an HPV 45 positive result mean?

HPV 45 is associated with cervical adenocarcinoma more than many other non-16/18 genotypes. A positive screening result is not a diagnosis of glandular precancer or cancer.

HPV 45 positive but Pap smear normal

In 2025 extended-genotyping guidance, HPV 16/18-negative HPV 45-positive results are triaged with cytology or approved dual stain rather than automatically following the HPV 18 pathway.

What if it is positive again?

Persistent HPV 45 deserves attention, but repeat positivity still must be interpreted with cytology, colposcopy and previous history.

How should grouped laboratory results be read?

If a laboratory reports 18/45 together, the result cannot be interpreted as HPV 45 alone unless the assay separately excludes HPV 18.

Is HPV 45 covered by vaccination?

HPV 45 is included in the 9-valent vaccine.

When can colposcopy or biopsy become relevant?

Colposcopy and biopsy decisions are based on the genotype, cytology or dual-stain result where appropriate, previous screening, prior CIN2/3 treatment and the clinical context. HPV genotype alone is not a biopsy diagnosis.
